If you suddenly ended up in the hospital, would anyone know your pets were home alone? For people who live alone, especially, a simple card can make sure a beloved dog or cat is fed and safe. This free printable card lists who to call, and the care instructions they'll need when they get there.

How to use your card
- Ask your contacts first. Make sure two people have agreed to help, and that at least one lives nearby.
- Cut out the top section and keep it in your wallet next to your ID, where paramedics and hospital staff will see it.
- Post the full page on your fridge or inside your front door for whoever comes to help.
- Keep keys private. Give a spare key only to someone you trust, and don't write where a key is hidden on the card.
- Update it when your pets' food, medicines, or your contacts change.
Pair it with our Emergency Contact and Medical Information Sheet so the people who help you have everything in one place.
