A complete, up-to-date list of your medicines is one of the simplest ways to stay safe. It helps doctors, pharmacists, and paramedics avoid dangerous mix-ups, and it saves you from trying to remember every name and dose on the spot. Fill in this free list, print it, and keep a copy in your wallet and one on the fridge.

Tips for keeping your list
- Include everything you take, including vitamins, supplements, herbal remedies, eye drops, inhalers, and over-the-counter medicines like pain relievers or sleep aids.
- Do a "brown bag" check once a year. Bring all your medicine bottles to your doctor or pharmacist and ask them to review your list. It's free and can catch duplicates or interactions.
- Update it whenever something changes, and write the date at the bottom.
- Keep copies where they'll help: one in your wallet, one on the fridge, and one with a family member or caregiver.
